HSCNI BSO Patient Information Directory: This directory of the BSO website has a number of helpful links and documents for patients looking to register with a GP Practice, change their GP Practice, or get a new medical card issued.
Patient Services website: Patients registered to use online Patient Services can login here and access their account to book and cancel appointments, request repeat prescriptions, and make administrative requests.
Self Certification Sickness Form: Use this form to self certify your absence from work due to poor health. You can self-ceertify for the first 7 days of absence from work with out needing a sick note / fit note from your GP.

Surgery opening hours

Connswater Family Practice is open Monday - Friday from 8.30am - 6.00pm. We close at weekends and on bank holidays.

Online patient services

If you have registered to use Patient Services, you can use it to book and cancel appointments, order repeat prescriptions and update your personal details online.

Online triage and consultations

The eConsult service allows you fill out a form and attach photographs to get advice from your GP and make administrative requests such as asking for a sick note or test results. This should not be used for urgent requests of a clinical nature.
